We recommend families and staff sign up for in-school testing. Providing consent using the forms below allows individuals to take a PCR test at school and also participate in the Test to Stay Program if they are ever identified as a close contact and need to quarantine. To sign up for School Testing, you must fill out BOTH the state form and a form from your school.
